We appreciate that each cat is an individual, with it's own likes and dislikes, please give us as much information as you can to ensure your cat feels at home. All fields except the last are required.

I hereby authorise Lesley Groves or Leslie Groves of Inglenook Cattery to seek veterinary treatment on behalf of my cat(s) named above and to consent on my behalf to any necessary treatment/operations.

I undertake to be responsible for the cost of any consultations/treatments/operations or other veterinary costs incurred and to reimburse the cattery for monies paid to veterinary practices on my behalf.

As owner of the above cat(s) I agree to the above terms which apply to this and all subsequent stays at Inglenook Cattery
